OPTIMALISASI PROGRAM KUNJUNGAN RUMAH DALAM MENINGKATKAN AKSES DAN PEMANFAATAN LAYANAN KESEHATAN PRIMER DI JAMBLANGAN SLEMAN

Abstract

Primary Health Care Integration (PHC Integration) is a key component of Indonesia’s health system transformation that emphasizes family-centered promotive and preventive services. One strategy to improve access to and utilization of primary health care services is the implementation of a home visit program. This community service project aimed to optimize the home visit program to improve access to and utilization of primary health care services in Jamblangan, Seyegan, Sleman Regency. A participatory approach was applied through four stages: preparation, community education, home visit implementation, and monitoring and evaluation. The program involved 20 households with a total of 56 family members. Activities included education on Primary Health Care Integration, Clean and Healthy Living Behavior (CHLB), balanced nutrition, the utilization of Posyandu and Community Health Centers (Puskesmas), and basic health screening. The program improved community awareness of the importance of primary health care services and encouraged greater utilization of Posyandu and Puskesmas. Screening identified five participants with hypertension, five with diabetes mellitus, and two active smokers, while no obesity cases were found. The home visit program also strengthened communication between Posyandu cadres and families and supported early detection of health risks.
